What are your fees?
*Oregon EMDR is an in-network provider for Providence Health Plans and Pacificsource Commercial plans but NOT Pacificsource Community Solutions, the OHP product.
*Oregon EMDR does NOT offer out-of-network billing.
*All clients are ultimately responsible for the cost of services with Oregon EMDR and payment can be made via HSA card, zelle, cash, check, credit card or PayPal.
*Please note that many isurance plans offer an out-of-network benefit and you may be eligible for partial or full reimbursement of out-of-pocket healthcare expenses paid to Oregon EMDR. Please contact your insurer to find out about your out-of-network benefit and information about submiting a "super bill" for reimbursement of expenses.
** Oregon EMDR Standard rates with insurance billing (Providence Health Plans and Pacificsource Commercial Plans only): **
$200 per 53-55 minute session (90791, 90837, 90847)
$165 per 38-52 minute session (90834)
** Oregon EMDR Self-pay Standard rates without insurance billing**
- $165 per 45-55 minute session (90791, 90837, 90834)
- $350 per 150 - 180 minute Extended Session (This rate will increase to $495 on 10/1/24)
** Oregon EMDR Self-pay Prepaid Reduced Rate without insurance billing: **
Clients may prepay $1800 for 12 individual therapy sessions.
Prepay rate breaks down as follows:
$150 per 45-55 minute session (90791, 90837, 90834)
Pro Bono services are available to 1 client at a time through the Returning Veterans Project.
Oregon EMDR operates in compliance with the Federal "No Surprises Act", as well as, the State Statutes of Illinois and Oregon regarding fair billing. How do I log into a telehealth session?
After you schedule a telehealth session with Susan, she will send you a link to the telehealth platform through the HIPAA-secure email feature of the client portal. This link will be unique to you and can be used for any and all scheduled telehealth sessions.
What is your grievance policy?
It is the policy of Oregon EMDR to treat all clients with fairness and professionalism and to strive for excellence in providing services to clients. Oregon EMDR policy provides clients and their families or legal guardians with the opportunity to express a problem or grievance related to the quality of services. If you feel you have been treated unfairly, unprofessionally or feel that your rights have been breached, the following procedure should be used.
Oregon EMDR grievance procedure is designed to provide a means for those applying for Oregon EMDR's services and clients receiving services to bring a grievance to the attention of Oregon EMDR and to reach a speedy resolution. Oregon EMDR has a strict policy prohibiting retaliation in any form against anyone who files a grievance.
A grievance is defined as any situation or condition that a client thinks is unfair, unjust or inequitable. In addition, if a client merely states they want to file a grievance, a grievance should be completed. Under this Client Grievance Procedure, you should submit a grievance in the following sequence:
If you have a grievance, the concern can be discussed with an Oregon EMDR staff. If you decide to speak to an Oregon EMDR staff and an agreement cannot be reached, you should proceed to the next step of this grievance procedure. You can also file a grievance without any discussion and proceed to the next step. Grievance forms can be found at the following:

If the matter has not been resolved to your satisfaction, you may choose to discuss your concerns with any supervisor.
Once notified in writing, the Director will initiate an investigation within 14 calendar days and provide an acknowledgment to you within 21 calendar days.
The Director will report the outcome of the complaint investigation to you within 30 calendar days after the complaint is received.

In circumstances where the matter of the grievance is likely to cause harm to the individual before the grievance procedures are completed, the individual or guardian of the individual may request an expedited review. The Director will review and respond in writing to the grievance within 48 hours of receipt of the grievance. The written response shall include information about the appeal process.
A grievant, witness, or staff member of a provider will not be subject to retaliation by a provider for making a report or being interviewed about a grievance or being a witness. Retaliation may include but is not limited to dismissal or harassment, reduction in services, wages, or benefits, or basing service or a performance review on the action.
The grievant is immune from any civil or criminal liability with respect to the making or content of a grievance made in good faith.
Individuals and their legal guardians may appeal entry, transfer, and grievance decisions as follows:
- If the individual or guardian is not satisfied with the decision, the individual or guardian may file an appeal in writing within ten working days of the date of the program administrator's response to the grievance or notification of denial for services. The appeal shall be submitted to the Division;
- If requested, program staff shall be available to assist the individual;
- The Division shall provide a written response within ten working days of the receipt of the appeal; and
- If the individual or guardian is not satisfied with the appeal decision, they may file a second appeal in writing within ten working days of the date of the written response to the Division Director.
This REPORTING AND PROCEDURE FORM is to be completed if you wish to make or file a grievance or complaint. You may also ask someone else who is acting with your knowledge and consent to write or express the grievance. (You may file this report anonymously, without your name, however it may make it more difficult for Oregon EMDR to address the matter.)
Use the Trainee and Consultee Grievance Form if you are a training participant or consultee.
What is your refund policy for training and consultation?
Oregon EMDR, LLC strives to offer flexibility and consideration to purchasers of Training and Consultation products. Details of Oregon EMDR’s refund policy are detailed below.
All refund requests must be made in writing. Requests can be submitted to Susan Wendelborg at susan@oregonemdr.com or to Oregon EMDR 1863 Pioneer Pkwy E, #441, Springfield, OR 97477.
** Single Day or Partial Day Trainings, Workshops and Webinars**
- Full refunds are available to purchasers of single-day trainings, workshops and webinars if submitted in writing at least 48 hours prior to the start of the event.
- No refunds will be offered under any circumstances for single or multi-day trainings, workshops and webinars after the start of the event.
** 6-Day EMDR Basic Training Refund Policy **
All cancellations must be requested in writing from the email used to register and will be confirmed in writing by Oregon EMDR, LLC.
Any cancellation request made more than 30 days prior to the training will receive a refund minus $150 administration fee. 30 days or less from the training no refunds will be made and You may transfer to a different training location, if space allows for a $150 administration fee. This includes cancellations due to weather, related issues, or any other reason.
Note: There will be no refunds, under any circumstances, for withdrawals after the beginning of the training program.
Once the training starts there are no transfers or refunds allowed. In the case of a bona fide emergency, illness or other issue, Oregon EMDR, LLC may allow the training dates to be made up by participating with another cohort, if space allows and at the sole discretion of Oregon EMDR, LLC for $250 per day missed. Any partial day missed will be counted as a full day. Missing more than one day or partial day could result in immediate dismissal from the training program and forfeiture of all tuition paid.
If a participant does not attend the training there will be no refunds, transfers or make-up allowed.
If You are more than 15 minutes late or leave more than 15 minutes prior to the end of the training day, Oregon EMDR, LLC may allow you, at their sole discretion, to make up the portion of what you missed with the trainer at their Standard Consultation Rate of $150 per hour. A minimum of 30 minutes will be required to be purchased to make up the time.
There is no partial credit given for any portion of the training completed if You do not complete the entire training.
Oregon EMDR, LLC is not responsible for any damages or expenses, including travel or otherwise due to any cancellations.
If the training is canceled for any reason you will be notified and issued a refund.
EMDR Certification Package
Full refunds are available to purchasers of the EMDR Certification Package if submitted in writing prior to ALL of the following circumstances:
- The purchaser being given access to any on-demand recorded content included in the purchase price of their Certification Package
- The purchaser attending consultation hours
- The purchaser attending a training, workshop or webinar that is included in the purchase price of their Certification Package
Partial refunds available for purchasers of the EMDR Certification Package within the following guidelines:
- The cost of all completed consultation hours at the standard rate of $150 / hour will be deducted from the refunded amount
- The cost of all recorded content the purchaser has been given access to will be deducted from the refunded amount at the standard FULL item purchase price
- The cost of all training, workshop or webinars attended will be deducted from the refunded amount at the standard FULL item purchase price
- Oregon EMDR will provide documentation of completed consultation hours to purchasers of the Certification Package requesting a refund but NOT a letter of recommendation for EMDR Certification
